By signing below, I hereby take total responsibility for my own safety and that of any minors under my care during BMC events, and release the BMC and any officer, contractor, lecturer or member thereof from all legal responsibility for injuries, accidents or illness during or as a result of any mushroom identification, field trip, meeting or dining event sponsored by the BMC.
